In addition to an increased prevalence of chronic disease, functional decline and functional limitations account for 64% of emergency department visits in adults 65 and older.&nbsp;With decreasing time providers have to spend with patients, integrating wearable activity monitors (such as an Apple Watch or Fitbit) into the healthcare setting for older adults to be used to remotely monitor and assess (1) changes to patients functional status, (2) adherence to rehabilitation programs, and (3) early referral and intervention when clinically significant changes are detected.&nbsp;Upon implementation, digital biomarkers can be defined and deployed to reduce risks associated with functional limitations and improve effectiveness of clinical care among the older adult population. However, little is known about how to efficiently and effectively implement wearable devices into an older adult healthcare setting, such as the needs and preferences of the target population for accessing and using these devices.</p> <p>Therefore, we propose to use a patient-centered approach to determine the needs and preferences of older adults to integrate wearable devices into a healthcare setting. Completion of this study will provide the knowledge necessary to design implementation strategies for integration of wearable devices into healthcare as a means to remotely monitor older adults health 秘密研究所</strong>, between the ages of 18-89 years, have a smartphone,&nbsp;and have a diagnosis of one of the following conditions: Parkinson鈥檚 disease, Multiple Sclerosis, stroke, post-Covid, lower extremity pain, back pain, or mild TBI/concussion. Unfortunately, there鈥檚 currently no easy way for physical therapist to assess how well their patients are following along with the at-home exercise programs they鈥檝e been given.</p> <p>The popularity of wearable devices like smart watches and fitness trackers have provided an opportunity for physical therapists to gain more objective information about their patient鈥檚 everyday exercise and physical activity. Recently, medical facilities have begun using online platforms to secularly share that information with their health care providers. These services, called Remote Therapeutic Monitoring systems, are cloud-based platforms that enable clinicians like Physical Therapists to better guide their patient鈥檚 adherence to at home health recommendations.</p> <p>The purpose of this pilot investigation is to determine the helpfulness, feasibility, and effectiveness of using a Remote Therapeutic Monitoring system to help physical therapists assess their patient鈥檚 adherence to their prescribed physical activity and exercise plans and assist clinical care, over the course of 6 months.</p> </div> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <h1><strong>Our Remote Therapeutic Monitoring System</strong></h1> <p>We鈥檝e partnered with Datos Health to provide a group of Shirley Ryan 秘密研究所 Physical Therapists a Therapeutic Monitoring platform for participating patients. Our Datos platform shares the patients鈥 daily step count, recorded workouts and heart rate during workouts with their treating therapist. This allows the therapists to stay up to date on their patient鈥檚 adherence to their prescribed activity goals and reach out to provide guidance. The purpose of this phase was to describe barriers and facilitators to using digital health technology in PT.</p><p>Results of the interviews suggest that clinicians should simplify digital health technology in PT to increase use by people with PD. We audited the initial session documentation of 30 historic control and 30 participants in the study. All of these sessions occurred in three outpatient rehabilitation clinics with individuals involved in the program for people with early stage Parkinson鈥檚 disease.</p><p>The PTs had 100% adherence to documenting self-reported baseline exercise for PwP. Implementation aimed to promote the use of DHT and a behavior change approach for exercise adherence in a group of PwP. &nbsp;Implementation strategies included interactive training, facilitation, documentation guidance and clinical resources/education for use with patients.</p><p>Trained PTs were more likely to document DHT as a tool for exercise monitoring after training, especially smart phones, fitness apps and wearables. PwP received an initial consultative PT assessment, follow-up DHT support as needed by a research facilitator, and a follow-up research evaluation at 6 months.&nbsp; PwP and PTs completed exit interviews about their experiences. &nbsp;</p><p>Recorded frequency and intensity of aerobic and strengthening exercises increased in PT documentation. The number of PwP reporting using DHT 鈮5 days/week increased from 11 to 18 and function remained stable, but there was no significant change mean minutes of exercise per week. PwP exit interviews suggest higher adoption of DHT for those who were initially more comfortable or eager to learn technology. PT exit interviews suggest that they value using DHT and scaled DHT recommendations to other patient populations. &nbsp;</p><p>These findings suggest that PT education and engagement can positively impact patient-reported use of DHT and facilitate increased exercise in PwP. With this intervention delivery, patients remained functionally stable through 6 months. &nbsp;</p><p>Incorporating DHT into physical therapy practice may be valuable to improve exercise adherence for paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>People with Parkinson's disease may fall due to issues with gait and balance or weak muscles. Falling can result in serious injuries, such as a broken hip. After a fall, people may worry about falling again, resulting in less walking or exercising and a tendency to stay home instead of running errands or meeting friends. Personal airbags may reduce this worry and prevent additional injury. Smart airbags worn under clothes can sense when the user is falling, inflate quickly to cushion the hip and then gently deflate under body weight to reduce the impact of falling.</p><p>This pilot study will determine if using a novel smart airbag design works and if using airbags is realistic and helpful for people with Parkinson's. We expect that a smart, wearable airbag system that can accurately predict falls and quickly inflate will reduce hip fractures in people with Parkinson's. We also expect that wearing the airbags will make people less scared of falling, so they will be more active within their home and community.</p><p>This study design involves two steps: (1) improving the airbag system so it can accurately detect falls in people with Parkinson's and (2) testing whether the system reduces injuries caused by falling. A group of 25 people with Parkinson's will wear the airbag system for six months, while another group of 25 will not. Everyone will carry a smartphone that can detect and record falls and measure activity levels. Information from the phones will tell us if (1) the airbags reduced the number of hip injuries from falls, and (2) if people wearing the airbags were more active.</p><p>The next step will be to have more people test the airbag system to confirm the results from this initial trial and to see if there are any negative effects from using the airbags. No objective test for Parkinson鈥檚 exists. If we are able to detect the disease at an early stage, we might be able to intervene and prescribe interventions that will slow progression of the disease and help people maintain their quality of life.</p><p>We believe that smart computer programs will allow us to detect early signs of PD from videos of people doing normal activities, such as drawing, walking, reaching for a cup or folding towels. We may also be able to predict the stage of disease and see how it is progressing.</p><p>We plan to use videos of people with PD doing different things to develop sophisticated computer programs that can detect specific movement patterns that are only seen in people with PD. We will then see how well our computer programs can diagnose early PD, compared with medical and clinical professionals. Our system may enable early detection of PD. Those people could then receive assistance and medication 鈥 when available 鈥 to delay the progression of the disease.</p><p>In the future, we plan to use slightly more sophisticated cameras that can capture more information than normal video. Shirley Ryan 秘密研究所 Vocational Rehabilitation counselors provide specialized counseling on strategies to retain current employment or obtain new employment in people with disabilities or medical conditions. However, Vocational Rehabilitation services are hard for people with Parkinson鈥檚 disease to access.</p><p>This peer support group and education series expands the availability of Vocational Rehabilitation to people with PD who would otherwise not be able to receive services at Shirley Ryan 秘密研究所 due to living out of state or other travel barriers. Meetings are conducted over Zoom twice a month. Quarterly, expert speakers will present on topics such as disclosure, ADA and employee rights, and other employment-related topics. class="field-label visually-hidden">Body</h4> <p>Clinical practice guidelines recommend interdisciplinary rehabilitation for people with Parkinson鈥檚 disease (PD) by a team including occupational therapy (OT), speech therapy (ST), and physical therapy (PT). In early PD, proactive rehabilitation includes (1) baseline evaluations, (2) exercise prescription and self-management advice, and (3) addressing early impairments in walking, balance, voice, dexterity, and occupational performance. The purpose of this study is to describe the delivery patterns, patient characteristics, and baseline performance of people referred to proactive rehabilitation at Shirley Ryan 秘密研究所.</p> </div> <h3 class="section-header">Description of Study Activities</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>Retrospective data from electronic medical records were extracted manually. Patients were included if they were referred to the proactive rehabilitation program for PD.</p> </div> <h3 class="section-header">Preliminary Results</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>The first stage of this study focused on the utilization of occupational therapy (OT). Descriptive statistics were used to characterize patient demographics and OT measures including basic activities of daily living (BADL), instrumental activities of daily living (IADL), nine-hole peg test, grip and pinch strength. A total of 72 patients were evaluated in the following clinical combinations: 17 PT/OT/ST, 8 ST/PT, 3 OT/PT, 2 OT/ST, 0 OT, 4 ST, and 38 PT. 22 patients were seen in OT (73% male, 68 卤 11 years old). 77% of these individuals were in HY stage 1-2, and 73% were within 5 years of diagnosis. Most individuals (91%) reported requiring increased time for at least one BADL. Writing and keyboarding were problems for 45% and 32% of individuals, respectively. Average nine-hole peg test values were 33 seconds (L) and 34 seconds (R). Average grip and pinch strength measures were 61.7 lbs. (L), 61.8 lbs. (R), 17 lbs. (L) 17.8 lbs. (R), respectively. Eighteen patients attended one 90 minute session, 3 patients completed 1 additional visit, and 1 patient completed 5 additional visits. OT is the least common area of proactive rehabilitation. European clinical practice guidelines recommend physical therapy (PT) for people with Parkinson鈥檚 disease (PD) soon after diagnosis to provide education, physical activity advice, and individualized interventions when needed. However, PT is frequently under-utilized early and individuals aren't referred until gait and balance problems occur. The purpose of this administrative case study is to present the application of a proactive PT (PAPT) approach, to optimize participation in evidence-based exercise for PD, using implementation frameworks for the (1) implementation process, (2) determinants of implementation success, and (3) implementation evaluation.</p> </div> <h3 class="section-header">Description of Study Activities</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>The PAPT program targets people with early stage PD before the onset of significant gait and balance dysfunction. The program uses a shared decision-making model to promote long-term maintenance of independent exercise. The Knowledge-to-Action Framework was used by champions to assess gaps between knowledge and current practice as well as synthesize current knowledge regarding exercise in PD. Implementation barriers were addressed using the Consolidated Framework for Implementation Research.</p> <p>The PAPT program was initiated in January 2016 and evaluated using the RE-AIM (Reach, Effectiveness, Adoption, Implementation, Maintenance) framework with mixed methods. Project 1 has been completed and accepted for publication.</p> <p>A second cycle through the Knowledge-to-Action Framework cycle was initiated in 2018 related to the spread and sustainment of the PAPT program through a regional rehabilitation system of care. After the first session, individuals followed 1 of 4 care paths to provide individually tailored intervention. Following PAPT, the number of participants reporting regular participation in aerobic, strengthening, flexibility, and balance exercise increased. Participants reported a median of 140 of aerobic exercise per week. Implementation barriers included location, insurance coverage, and difficulty scheduling long-term follow up visits. Participants reported physical and emotional benefits of the program. Referral rates increased from targeted referral sources by 55-163%.</p> </div> <h3 class="section-header">Project 2 Preliminary Results</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>In the program鈥檚 first year, 26% of referred individuals (10 of 38) did not attend the urban program due to barriers due to lack of value of the program given interest, transportation, or cost barriers. During the first year of program expansion, our PTs reached 61 patients, and we are on target for additional growth in 2019.&nbsp; Key barriers in suburban programmatic development include slow referral growth, staff turnover, and scheduling communication. Unique benefits of implementing a program the smaller clinical settings include completion of patient-reported outcome measures, better consistency of therapists with smaller programs. The most potent facilitator of program sustainment were administrative and leadership support. geographic location, appropriately trained professionals, and facility space. Many people with PD and their exercise leaders would like to know how these community exercise programs are helping their physical function. The purpose of this study is to develop a testing battery that would allow for community exercise professionals to better understand the impact of their classes.</p> </div> <h3 class="section-header">Aims</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>(1) Develop a feasible test battery that can be administered in a short time by community exercise leaders and easily understood by participants with Parkinson's disease (PD) and develop a secure platform to tract the progress and outcomes of people with PD doing exercise in the community.</p> <p>(2)&nbsp;Apply the measurement plan twice over a 2-4 month time frame to people with PD in exercise classes in urban, suburban, and rural areas.</p> <p>(3) Use the battery for quality improvement of exercise classes by providing feedback to course instructors and participants with PD.</p> </div> <h3 class="section-header">Description of Study Activities</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>Through this project we aim to reduce the number of tests to improve efficiency of tests chosen so tests take no more than 20 minutes to complete, and/or can be completed in a group setting. After developing the test battery, we will train necessary personnel on how to conduct the test in an efficient manner. The test battery will then be consistently administered multiple times over a 6-month period across the 15 core program classes, as well as additional classes and sites that are recruited and identified. Upon completion of the testing, the exercise professionals will use the data for quality improvement. We will determine if there is a differential benefit to the classes offered and/or if the classes targeted the area of exercise that was intended. Upon completion of this project, we will create educational tool and disseminate our findings and test battery to exercise professionals across the Chicago-land area using the Parkinson's Foundation outreach network.</p> </div> <h3 class="section-header">Preliminary Results</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>Over the past year, we have conducted semi-consistent testing of a variety of outcome measures for people who attend one-on-one functional fitness appointments and/or PD specific classes.</p> <p>The benefits of our pilot program are evidenced by participants and their care partners who stated: (1) "Thanks for your detailed and positive reports on [husband's] progress; we're both delighted, of course!" (2) "My husband is happy to hear your report of his work with you, and your good record-keeping on his testing results is also helpful." (3) "Having regular testing shows me that coming here, I'm not regressing or maintaining, it's [exercise] actually helping me improve. It keeps me optimistic about my future, because these testing numbers are showing me I am beating the odds."</p> <p>The consistency of the testing also allows our exercise physiologists to tailor participants' workout structures based on changes in outcome measures, and then specifically target different areas of exercise (such as balance, strength and/or aerobic) leading to better improvement However, barriers to exercise are significant among people with PD. This study utilizes a mixed methods approach to identify factors and life-changes associated with changes in exercise patterns among people with PD. The final products of this study include resources developed to help people with PD sustain their exercise.</p> </div> <h3 class="section-header">Aims</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>(1) Identify disease-related factors associated with people who increase their exercise and people who decrease their exercise.</p> <p>(2) Identify life changes and processes associated with exercise increases and decreases.</p> <p>(3) Develop an implementation toolbox to facilitate exercise adherence and sustainment for people with PD.</p> </div> <h3 class="section-header">Description of Study Activities</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>Using data from the Parkinson鈥檚 Foundation Quality Improvement Initiative (PF-QII) we categorized participants into 鈥渆xercise increasers鈥 and exercise decreasers.鈥 We compared groups鈥 measures of mobility, quality of life, hospitalization, and falls. We then conducted phone interviews on a subset of participants from Northwestern to assess personal factors that influence the ability to increase and sustain exercise.</p> </div> <h3 class="section-header">Results</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>A change of 1.5 hours per week was identified as being associated with better health for increased exercise, and poorer health for decreased exercise. Overall, exercisers are younger, more recently diagnosed, and healthier.
